Attach a copy of your Benefits Summary Letter from Veterans
Affairs. You may also access this information from
https://www.ebenefits.va.gov
Do not send in a copy of your VA Decision Letter as
evidence of a service-connected disability. This letter
contains HIPAA restricted information which we cannot
accept.

Attach a copy of information showing residency in Oregon
(copy of driver's license, state ID or envelope with your
Oregon address).
Foster Parents required documentation

Attach a copy of your valid Certificate of Approval to
Maintain a Foster Home in Oregon. The pass will be issued
to the name on the certificate. It is valid until the
foster home certificate expires.

Attach a copy of information showing residency in Oregon
(copy of driver's license, state ID or envelope with your
Oregon address).
Guardian and Adoptive Foster Parents required documentation

Attach your Adoptive or Guardian Family Fee Waiver Program
Verification Letter issued by DHS identifying you as an
adoptive or guardian family. DHS provides these letters; we
do not have them. Call DHS in Salem at 503-947-1134 for
more information.

Replacement card fee $10.00
Upon submission of this request you will be redirected to a
checkout screen to collect the replacement fee. The replacement
card will only be issued upon successful completion of this
